At Bodyworks Physical Therapy in Fargo, we’re excited to support patients at every stage of their health journey. And recently, we’ve noticed something worth talking about.
Some patients, particularly women, have shared that during periods of rapid weight loss, they’ve experienced new or increased low back, hip, or SI joint discomfort. This doesn’t happen to everyone, and it doesn’t mean something is wrong, but if you’re noticing changes in how your body feels, you’re not alone.
Why This Might Be Happening During GLP-1 Related Weight Loss?
When weight loss happens quickly, the body sometimes needs time to adjust. Several factors may be working together:
1. Changes in joint support
Body weight (including healthy fat tissue) provides a certain amount of passive support around joints such as hips, pelvis, and low back. When weight decreases rapidly, joints may temporarily feel more mobile until surrounding muscles adapt.
2. Muscle loss alongside fat loss
Some people lose lean muscle mass during rapid weight loss. Muscles play a key role in joint stability – especially around the core, hips, and pelvis. If strength and coordination don’t keep pace with weight changes, discomfort or instability can develop.
3. Hormonal shifts and connective tissue
Hormones, including estrogen, influence connective tissue and joint stability. Because hormones are affected by metabolism and body fat, some individuals notice changes in joint comfort during rapid weight loss. This may help explain why women report these symptoms more often.
Many patients describe their symptoms like this:
“This feels similar to how my body felt during pregnancy or postpartum.” And that comparison actually makes sense. During pregnancy, the hormone relaxin increases ligament flexibility, especially around the pelvis, to support childbirth. While GLP-1 medications do not directly increase relaxin, rapid metabolic and hormonal shifts during significant weight loss may create similar effects on connective tissue in some individuals. Research is still evolving, but clinically, the pattern can resemble pregnancy or postpartum type joint changes.
4. The nervous system needs time to recalibrate
As body size, weight distribution, and loading change, the nervous system must relearn how to control movement and posture. During the adjustment period, joints may feel “off,” even without an injury.
Common Symptoms We’re Seeing With GLP-1 Medications
Patients using GLP-1 medications may report:
- Pelvic, hip, or low back pain
- SI joint discomfort
- A feeling of looseness or instability
- Pain that appears without a clear injury
Again, this doesn’t happen to everyone and is not dangerous but it is something more often.
